Occipital neuritis, also known as occipital neuralgia, is a distinct type of headache characterized by sharp, shooting, or jabbing pain. This pain starts at the base of the skull and radiates along the greater and lesser occipital nerves, which run over the back of the head and sometimes to the sides, forehead, and behind the eyes.
- The Anatomy of the Problem: The occipital nerves emerge from the upper cervical spine, specifically from the C2 and C3 nerve roots. They must navigate through a complex network of muscles, fascia, and ligaments at the base of the skull, including the powerful suboccipital muscles. When these nerves become compressed, irritated, or inflamed at any point along their path, they can cause the characteristic pain of occipital neuritis.
- Common Causes: The irritation can stem from various sources. Whiplash from a car accident, chronic poor posture (especially “text neck”), or repetitive neck strain can lead to muscle tightness and inflammation that entraps the nerves. Other causes include structural misalignments in the upper cervical vertebrae, osteoarthritis, or direct trauma to the back of the head. From a functional medicine perspective, systemic inflammation can also contribute by sensitizing the nerves and making them more susceptible to irritation.
- The Patient Experience: Patients often describe the pain as feeling like an electric shock or a sharp jab. The scalp can become exquisitely tender to the touch, to the point where even brushing hair or resting the head on a pillow is unbearable. Unlike migraines, which are often throbbing and may be accompanied by nausea and light sensitivity, occipital neuritis pain is typically sharper and more localized along the nerve’s pathway.
The Diagnostic Process: Pinpointing the Pain
Accurate diagnosis is the foundation of effective treatment. When a patient presents with symptoms suggestive of occipital neuritis, my first step is to perform a thorough physical examination. A key diagnostic sign is reproducing pain when I apply gentle pressure over the occipital nerves at the base of the skull. This is known as Tinel’s sign.
As part of my assessment, I carefully palpate the suboccipital region to locate the precise points of maximum tenderness. For instance, in a recent case, I identified two distinct tender points along the patient’s hairline. To accurately target these areas during the procedure, I used a simple but effective technique: marking the skin.
- Locating the Epicenter: I asked the patient, “Is that hurting right there?” Upon their confirmation, I knew I had found an active trigger point directly over the nerve.
- Marking the Site: Using the tip of a retractable ballpoint pen (without the ink extended), I made a small indentation in the skin. This physical mark is more reliable than an ink mark alone, which can be wiped away during skin preparation. I then added a small ink dot just below the indentation for visual reference.
- Repeating the Process: I repeated this process for the second tender spot, ensuring both pain sites were clearly marked for the upcoming injection.
This meticulous localization is critical. The occipital nerves are small targets, and precision is paramount to delivering the medication exactly where it is needed most.
The Occipital Nerve Block: A Targeted Intervention
For acute, severe pain from occipital neuritis, an occipital nerve block can provide rapid, significant relief. This procedure involves injecting a combination of a local anesthetic and a corticosteroid directly around the inflamed occipital nerve.
The Science Behind the Injection
- Lidocaine (The Anesthetic): This is a fast-acting numbing agent. Its primary purpose is to provide immediate pain relief by blocking pain signals from the nerve to the brain. When I inject the lidocaine, the patient often feels the intense pain subside within minutes. This immediate feedback also serves a diagnostic purpose: if the pain disappears, it confirms that the targeted nerve was indeed the source of the problem.
- Cortisone (The Anti-Inflammatory): This powerful steroid medication reduces inflammation. While lidocaine offers short-term relief, the cortisone addresses the underlying issue—the inflamed nerve. By calming the inflammatory process, the cortisone provides longer-lasting relief, often for several weeks or even months. This “window of opportunity” is crucial, as it allows us to implement other therapies, such as chiropractic care and physical rehabilitation, while the patient is comfortable.
Performing the Procedure
Once the tender areas are marked and the skin is cleansed with alcohol, I can administer the injection. Safety is my top priority.
- Needle Selection: I use a fine-gauge needle (25-gauge, 1-inch) to minimize discomfort during the injection.
- Aspiration: Before injecting the medication, I gently pull back on the syringe plunger—a technique called aspiration. This critical safety check ensures the needle tip is not inside a blood vessel. If blood were to enter the syringe, it would indicate improper placement, and injecting the steroid into a blood vessel could cause systemic side effects. In this case, no blood returned on aspiration, confirming the needle was correctly positioned in the soft tissue surrounding the nerve.
- Injection: I then slowly and carefully injected half of the medication at the first site, bathing the irritated nerve in the anesthetic and anti-inflammatory solution. I repeated the exact same process at the second marked location.
- Post-Injection Massage: After the injections, I gently massaged the area to help disperse the medication throughout the surrounding tissues, ensuring it reaches the entire inflamed segment of the nerve.
Immediately following the procedure, I re-palpated the areas. The patient who had previously winced in pain now reported feeling only pressure. When asked to compare the feeling, they confirmed it was significantly better. This immediate positive response is a hallmark of a successful occipital nerve block.
The Role of Integrative Chiropractic Care in Long-Term Healing
While an occipital nerve block is highly effective for acute pain, it is not a standalone cure. It opens the door to more comprehensive, long-term solutions. That is where the power of integrative chiropractic care comes in. Inflammation didn’t appear in a vacuum; an underlying mechanical or functional issue caused it and must be addressed to prevent recurrence.
Chiropractic Adjustments for Cervical Alignment
As a chiropractor, I focus on the biomechanics of the spine. Misalignments, or subluxations, in the upper cervical vertebrae (the atlas and axis) can directly impinge on the C2 and C3 nerve roots, leading to occipital nerve irritation.

- The Technique: Depending on the patient’s condition and anatomy, I may use the Activator method, diversified adjustments, or upper cervical-specific techniques. By correcting the structural source of the problem, we help the body heal itself.
Soft Tissue and Myofascial Release
The occipital nerves are intimately related to the suboccipital muscles. Chronic tension, trigger points, and fascial adhesions in these muscles can physically constrict the nerves.
- The Approach: We utilize various soft tissue techniques to address this muscular component. These may include:
- Myofascial Release: Applying sustained pressure to release tightness in the fascia, the connective tissue that envelops the muscles.
- Trigger Point Therapy: Applying direct pressure to muscle “knots” to release tension and improve blood flow.
- Instrument-Assisted Soft Tissue Mobilization (IASTM): Using specialized tools to break down scar tissue and fascial restrictions.
By releasing this muscular tension, we create more space for the occipital nerves, reducing compression and allowing them to heal.
Therapeutic and Rehabilitative Exercises
Lasting recovery requires empowering the patient to take an active role. I prescribe specific therapeutic exercises to correct postural imbalances and strengthen the deep neck flexor muscles that support the head and neck.
- Stretching: Gentle stretches for the upper trapezius and levator scapulae muscles help relieve chronic tightness.
- Strengthening: “Chin tuck” exercises are fundamental for strengthening the deep cervical flexors and correcting the forward-head posture that so often contributes to occipital neuritis.
When performed consistently, these exercises help create a stable, resilient musculoskeletal environment, making future nerve irritation far less likely.
