Functional medicine does not mean that every patient needs a long list of laboratory tests, supplements, or restrictive diets. Testing should be based on the patient’s history, symptoms, examination, medications, and clinical needs.

PRP: Platelet-Rich Plasma
PRP is prepared from a sample of the patient’s own blood.
The blood is placed in a centrifuge, which separates and concentrates the platelets. The prepared plasma is then injected into the selected area under appropriate medical conditions.
Platelets contain growth factors and signaling proteins involved in clotting and tissue repair. PRP may be considered for certain problems involving:

PRP is not one standard product. Its contents can change based on:

- Platelet concentration

This may help explain why patients and research studies do not always report the same results.
Some studies have reported improved pain and function following PRP. Other studies have not found an important difference compared with a placebo. For example, the RESTORE trial did not demonstrate a significant advantage for PRP over saline for knee osteoarthritis after 12 months (Bennell et al., 2021).
PRP should therefore be discussed as a possible treatment option, not as a guaranteed cure.
PFP: Platelet-Fibrin Products
PFP is a broad term used for products that combine platelets with a fibrin framework.
Platelet-rich fibrin is one example.
Fibrin is a protein involved in blood clot formation. It can create a soft framework that may keep platelets and healing signals near the treatment area.
PFP may be considered for selected:
- Tendon injuries
- Ligament injuries
- Muscle injuries
- Joint problems
- Areas requiring a slower release of platelet signals
PFP and PRP are related, but they are not identical. The preparation method affects the product’s thickness, cell content, fibrin structure, and possible clinical use.
Patients should ask what platelet product is being recommended, why it was selected, and what evidence supports its use for their condition.
MFAT: Microfragmented Adipose Tissue
MFAT uses a small sample of the patient’s own fat tissue.
The tissue is usually collected through a small procedure. It is then cleaned and mechanically processed into smaller pieces before being placed into the treatment area.
MFAT may contain:
- Structural tissue
- Supportive cells
- Blood-vessel-related cells
- Signaling molecules
- Natural tissue framework
It may be considered for certain deeper joint problems, including some cases of knee osteoarthritis.
MFAT should not automatically be described as laboratory-grown stem-cell therapy. It is a mechanically processed form of the patient’s own adipose tissue.
Research remains mixed. One randomized trial reported greater improvement in knee pain with MFAT than with saline during one year of follow-up. Another blinded trial found that MFAT was not better than saline after two years (Barfod et al., 2025; Richter et al., 2025).
Patients should receive an honest explanation of the possible benefits, risks, cost, recovery period, and limits of the available evidence.
Epidural Spinal Injections
Epidural spinal injections are different from PRP, PFP, and MFAT.
A traditional epidural injection usually places a corticosteroid and anesthetic medication near an irritated spinal nerve. It may be considered when a disc problem, arthritis, or spinal narrowing causes:
- Sciatica
- Arm or leg pain
- Numbness
- Tingling
- Burning pain
- Nerve-root inflammation
- Reduced ability to walk or exercise
The main goal is to calm inflammation around the nerve.
An epidural steroid injection does not rebuild a damaged disc. It may create a period of reduced pain that allows the patient to sleep, walk, and participate more fully in rehabilitation.
A 2025 review from the American Academy of Neurology found that epidural steroid injections may provide modest short-term improvements for some people with cervical or lumbar radiculopathy. Long-term benefits are less certain.
Bioidentical Hormone Replacement and Recovery
Hormones affect:
- Muscle mass
- Bone strength
- Energy
- Sleep
- Metabolism
- Mood
- Sexual health
- Overall vitality
A true hormone deficiency may affect a person’s ability to exercise, maintain muscle, or feel well. When symptoms and laboratory findings support a diagnosis, hormone replacement may be considered under medical supervision.
However, hormone therapy should not be promoted as a direct treatment for a torn tendon, damaged ligament, worn joint, or injured spinal disc.
It is one possible part of whole-body health management.
Patients should also understand that the word “bioidentical” does not automatically mean a product is safer. Some FDA-approved hormone products are chemically identical to hormones produced by the body. Custom-compounded products may have additional concerns related to dosing, purity, and oversight.
The Endocrine Society recommends careful diagnosis, risk review, patient education, and ongoing monitoring for hormone treatment.
Can Injections and Chiropractic Care Be Combined?
Yes, they may often be combined when the patient has been properly screened, and the providers communicate.
Timing matters.
Immediately after an injection, the treated tissue may need protection. Forceful treatment directly over a recently injected area may not be appropriate.
Early chiropractic care may focus on:
- Gentle movement
- Nearby joints
- Posture
- Comfortable walking
- Muscle relaxation
- Reducing strain on the treated area
As healing progresses, treatment can shift toward:
- Range-of-motion exercises
- Isometric contractions
- Core stability
- Balance training
- Gradual resistance

- Long-term injury prevention

Pre-Procedure Screening and Safety
Regenerative procedures and spinal injections should not begin with the injection itself. They should begin with a medical evaluation.

Some clinicians recommend avoiding nonsteroidal anti-inflammatory drugs, or NSAIDs, around a PRP procedure because certain medications may affect platelet function.
Examples may include:
- Ibuprofen
- Naproxen
- Aspirin
However, the effects are not the same for every medication. A systematic review found that some drugs affect platelet function, while COX-2-selective NSAIDs may not need to be stopped before PRP (Kao et al., 2022).
Stopping aspirin or a blood thinner may be dangerous for a person with heart, stroke, or clotting risks. The prescribing clinician must approve any medication change.
Steroid use should also be reviewed. Patients should not suddenly stop oral steroids, inhalers, or other prescribed medications. The medical team must decide whether the timing or dose affects the planned procedure.
Understanding FDA Oversight
The term “regenerative medicine” is used for many different products. Not every product advertised online is approved, legally marketed, or supported by good evidence.
The FDA warns that many stem-cell, exosome, umbilical-cord, Wharton’s jelly, and related products have not been approved to treat orthopedic conditions such as arthritis, tendon injuries, disc disease, back pain, or joint pain.
Patients should ask:
- What exactly is being injected?
- Does it come from my own body?
- How is it processed?
- Is it FDA-approved or used under another regulatory pathway?
- What evidence supports this use?
- What are the known risks?
- Who will perform the procedure?
- What happens if there is a complication?
Clear answers are an important part of informed consent.
A Realistic Recovery Timeline
Regenerative treatment is usually not an instant fix.
Some soreness may occur during the first few days. Improvement may develop gradually over several weeks or months.
Recovery depends on:
- The diagnosis
- The severity of tissue damage
- The patient’s age
- General health
- Blood sugar control
- Smoking status
- Sleep
- Nutrition
- Activity level
- Adherence to rehabilitation
