Before we explore the barbotage procedure, it is crucial to understand the condition it treats: calcific tendinopathy. This disorder involves the formation of calcium hydroxyapatite deposits within a tendon. While it can occur in various locations, such as the patellar tendon in the knee or the gluteus medius tendon in the hip, it is most commonly diagnosed in the supraspinatus tendon of the shoulder’s rotator cuff.
These calcium deposits can be in different phases:
Patients in the resorptive phase often present with acute, excruciating shoulder pain that can disrupt sleep and daily activities. The goal of treatment is to alleviate this pain and facilitate the removal of the calcium deposit. Ultrasound-guided barbotage is a highly effective, evidence-based intervention designed to achieve precisely that. The image below shows a classic example: you can see the skin, the deltoid muscle, and the supraspinatus tendon. Within the tendon is a bright, white calcium deposit. Notice the dark acoustic shadowing beneath it; this is a key diagnostic marker on ultrasound, as the dense calcium blocks the ultrasound waves from passing through.
Barbotage, derived from the French word for “to splash” or “to bubble,” is a procedure of repeated injection and aspiration used to break down and remove these painful calcium deposits. The entire procedure is performed under real-time ultrasound guidance, allowing precise needle placement and ensuring both safety and effectiveness. By visualizing the needle, the calcium deposit, and surrounding anatomical structures, we can target the problem area while avoiding injury to healthy tendon tissue and nearby neurovascular structures.
The primary objective is to decompress the tendon by removing the calcific material, which immediately reduces the intense pressure and inflammation responsible for the patient’s pain. This technique is a powerful alternative to more invasive surgical options.
Two primary methods exist for performing barbotage, and the choice often depends on the practitioner’s preference and the characteristics of the calcium deposit.
This method creates a “washing circuit” to flush out the calcium.
The single-needle technique, which I will detail in the procedural walkthrough below, is also highly effective. It involves using a single, larger-gauge needle to both inject saline and aspirate the dissolved calcium. This method requires repeatedly repositioning the needle within the deposit to target different areas, injecting fluid to create a slurry, and then aspirating it back into the same syringe.

The first and most critical step is ensuring the patient is comfortable and the procedure is performed under sterile conditions.
Once the area is numb, the main part of the procedure begins.
After the barbotage is complete (a process that can take 5-10 minutes of active work), it is common to perform a final injection.

The barbotage procedure is a powerful tool for removing the primary pain generator, but comprehensive recovery requires addressing the underlying biomechanical and physiological factors that may have contributed to the tendinopathy in the first place.
