PRF and Chiropractic Care for Faster Injury Recovery

Abstract

Sports injuries can affect much more than a single muscle, tendon, or joint. An injury may also change the way a person walks, moves, trains, sleeps, and performs daily activities. For this reason, recovery may benefit from an approach that looks at both tissue healing and whole-body function.

Platelet-rich plasma (PRP) and platelet-rich fibrin (PRF) are biologic treatments made from a patient’s own blood. They contain concentrated platelets and natural signaling proteins involved in the body’s repair process. PRF is prepared differently from traditional PRP and forms a fibrin-rich structure that can hold platelets and other blood components within a three-dimensional matrix.

When appropriate, PRF or PRP may be combined with integrative chiropractic care, functional medicine, and rehabilitation. The platelet treatment focuses on the injured tissue, while chiropractic and rehabilitative care address mobility, joint mechanics, muscle balance, and movement patterns. This whole-person model fits well within an integrative wellness approach because it considers not only where the injury hurts, but also the physical and health factors that may influence recovery.

Understanding PRP, PRF, and “PFP”

Before discussing sports injury treatment, it is helpful to clarify the terminology.

PRP means platelet-rich plasma.

PRF means platelet-rich fibrin.

The phrase “platelet fibrin plasma,” sometimes shortened to PFP, may occasionally be used in casual discussions, but it is not the standard term most commonly used in medical literature. PRP and PRF are the more accurate terms when discussing platelet-based regenerative procedures.

Both treatments begin with a small blood draw.

The blood is placed into a centrifuge, which separates different blood components. A platelet-rich portion is then prepared for use in the injured area.

Platelets are well known for helping blood clot, but they also contain proteins and growth factors involved in tissue repair. These biological signals help cells communicate during the normal healing process.

Healthcare centers such as Yale Medicine, Johns Hopkins Medicine, and Hospital for Special Surgery describe PRP as an autologous treatment because it uses material taken from the patient’s own blood (Hospital for Special Surgery, 2024; Johns Hopkins Medicine, n.d.; Yale Medicine, n.d.).

What Makes Platelet-Rich Fibrin Different?

PRF is often called a newer generation of platelet concentrate.

One of its biggest differences is the way the blood is processed.

Some PRF methods use lower centrifugation forces and do not add an anticoagulant. This allows the patient’s natural clotting process to begin.

As the blood begins to clot, fibrin fibers form a three-dimensional network.

That network may hold:

  • Platelets
  • White blood cells
  • Growth factors
  • Proteins
  • Other cellular components involved in healing

The result may be a clot, membrane, gel-like material, or injectable form known as injectable PRF, or i-PRF.

Research suggests the fibrin structure can provide a temporary scaffold within which biological signaling continues over time (Grecu et al., 2019).

This does not mean PRF creates an entirely new tendon or ligament by itself. Instead, it may help support the environment in which the body’s natural repair processes take place.

PRF Is Not the Same as Stem Cell Therapy

PRF is sometimes described online as containing stem cells or functioning like stem-cell therapy.

That description can be misleading.

PRF is primarily a fibrin- and platelet-based blood product. It may contain several types of circulating cells, depending on how it is prepared, but this does not make it the same as a dedicated stem-cell treatment.

A more accurate way to explain PRF is that it provides a biological framework containing platelets, fibrin, signaling proteins, and other blood components that may help support the body’s healing response.

This distinction is important when discussing regenerative medicine because patients should understand exactly what type of treatment they are receiving.

How Does PRF Support the Healing Process?

Healing after a sports injury requires more than simply stopping pain.

The body must move through several stages of repair. These may include inflammation, cellular activity, tissue rebuilding, and remodeling.

Fibrin naturally plays an important role during early wound healing.

The fibrin matrix found in PRF may provide a temporary structure that helps keep useful biological components near the injured area.

Possible benefits of the fibrin network include:

  • Holding platelets near damaged tissue
  • Supporting gradual release of signaling molecules
  • Providing a temporary framework for healing
  • Supporting cell migration
  • Helping organize the body’s natural repair response
  • Maintaining biological activity around the treatment area

PRF does not guarantee that an injury will heal faster. Recovery still depends on factors such as injury severity, circulation, nutrition, sleep, activity level, metabolic health, rehabilitation, and how much stress is being placed on the injured tissue.

What Sports Injuries May Be Considered for Platelet Therapy?

PRP has been researched more extensively than PRF for orthopedic and sports medicine conditions.

Platelet-based treatments have been studied for injuries involving tendons, ligaments, muscles, joints, and cartilage (Milano et al., 2019; Patil et al., 2024).

Conditions that may sometimes be evaluated for PRP or PRF include:

  • Achilles tendinopathy
  • Patellar tendinopathy
  • Tennis elbow
  • Golfer’s elbow
  • Rotator cuff tendinopathy
  • Selected partial tendon tears
  • Certain ligament injuries
  • Chronic muscle injuries
  • Knee joint problems
  • Selected cartilage injuries
  • Slow-healing soft-tissue injuries

The evidence is not equally strong for every condition.

Some studies report meaningful improvement, while others show limited or inconsistent benefits. Differences in injury severity, injection technique, rehabilitation, and the way platelet products are prepared can affect outcomes (Cruciani et al., 2019).

PRF research in sports medicine is still growing. Reviews suggest that PRF may have useful orthopedic applications, but larger and better-standardized clinical studies are still needed (Costa et al., 2025; Grecu et al., 2019).

Why Sports Injury Recovery Should Be More Than an Injection

A sports injury is rarely only a problem at the exact point where pain is felt.

Consider an athlete with knee pain.

The knee may be injured, but the athlete may also have:

  • Weak hip muscles
  • Poor ankle mobility
  • Reduced balance
  • Abnormal running mechanics
  • Limited joint motion
  • Muscle imbalance
  • Compensation from an old injury

If these problems are not addressed, the healing tissue may continue to experience excessive or uneven stress.

This is why a whole-person wellness approach can be useful.

A regenerative procedure may focus on the damaged tissue, while integrative chiropractic care and rehabilitation address the movement patterns surrounding the injury.

Chiropractic care does not replace PRF, and PRF does not replace rehabilitation.

Biological Healing Meets Mechanical Recovery

A useful way to understand integrative sports injury care is to divide recovery into two connected parts.

Biological Recovery

This involves the damaged tissue itself.

PRP or PRF may be used to provide concentrated platelets and biological signaling compounds to the injured area.

The goal is to support the body’s natural repair environment.

Mechanical Recovery

This involves how the body moves.

Chiropractic care, rehabilitation, therapeutic exercise, mobility work, and progressive strengthening may help improve the way forces move through the joints and soft tissues.

Neither side should automatically be considered more important than the other.

A tendon may need biological support, but it also needs to regain the ability to safely tolerate movement and load.

Functional Medicine and Whole-Body Healing

Wellness-oriented care also considers internal health factors that may influence recovery.

A person may have a well-designed rehabilitation program but still struggle to recover if the body is not receiving proper support.

Functional and integrative medicine may consider factors such as:

  • Protein intake
  • Vitamin and mineral status
  • Blood sugar control
  • Sleep quality
  • Hydration
  • Inflammation
  • Hormonal balance when appropriate
  • Stress
  • Exercise recovery
  • Metabolic health

These factors do not replace orthopedic diagnosis or injury treatment.

Instead, they provide a broader picture of the health environment in which healing takes place.

For example, poor nutrition may limit the building blocks available for tissue repair. Poor sleep may affect pain sensitivity and recovery. Uncontrolled metabolic disease may also influence healing.

  • How can future injury risk be reduced?
