We used to perform dry needling, which involves inserting a fine, solid filiform needle into the trigger point without injecting any substance. They might be tempted to say, “No more!” By starting with less sensitive points and building trust, the patient remains more relaxed. This is a practical, patient-centered strategy that I find is crucial for success. I don’t want to argue with my patients; I want to partner with them in their healing.

The choice of what to inject is just as important as the technique itself. It does not damage the nerve but rather interrupts its ability to transmit pain. This provides prolonged pain relief that lasts long after the lidocaine has worn off.
- Why Sarapin Over Steroids?: Corticosteroids are potent anti-inflammatory drugs, but their use comes with potential downsides, especially with repeated injections. They can weaken tendons and cartilage, suppress the local immune response, and, over time, cause tissue atrophy (degeneration). From my clinical perspective, steroids are a powerful tool but should be used judiciously. Sarapin, on the other hand, offers a powerful anti-inflammatory and analgesic effect without these associated risks. It is an all-natural product that supports healing rather than suppressing the body’s systems. My experience has consistently shown that the combination of lidocaine and Sarapin yields a superior, more durable result than lidocaine alone or even lidocaine with steroids for this particular application. The one-to-one ratio of lidocaine to Sarapin provides a balanced formulation that addresses both the immediate pain and the underlying inflammation driving the trigger point’s persistence.
Customizing the Treatment: Depth, Dosage, and Anatomy
Effective TPI is not a one-size-fits-all procedure. It requires a deep, three-dimensional understanding of human anatomy and the ability to adapt the technique based on the specific muscle being treated and the individual patient’s body composition.
Injection Depth and Safety
A common concern for both patients and practitioners learning the technique is safety, particularly the risk of puncturing a lung (pneumothorax) when injecting in the thoracic region. This is where anatomical knowledge is paramount.
- Cervical and Upper Thoracic Regions: In the neck and upper back, the muscles are generally more superficial, and vital structures are close by. For these areas, I typically use a 30-gauge, 1-inch needle. The smaller gauge makes the injection more comfortable, and the 1-inch length provides a built-in safety margin. Even in a very thin individual, a 1-inch needle inserted perpendicularly is highly unlikely to reach the pleural space of the lungs. The layers of skin, subcutaneous fat, muscle (like the trapezius and rhomboids), and the rib cage itself provide significant protection.
- Lower Thoracic and Lumbar Regions: In the lower back, the muscles (like the quadratus lumborum and erector spinae) are much thicker and more robust. Here, we can safely use a slightly larger needle, such as a 5-inch needle, to ensure we reach the full depth of the trigger point, which may be located deep within the muscle belly. While a 1.5-inch needle carries a slightly greater theoretical risk in the thoracic area, especially in frail or elderly patients, it is perfectly safe in the thick musculature of the lumbar spine. Constant awareness of the underlying anatomy—knowing where the ribs are, the angle of their slope, and the location of the kidneys—is non-negotiable.
The key is to go back and constantly review your anatomy. This is a principle I stress to all healthcare professionals, whether they are in orthopedics, aesthetics, or pain management. I’ll be the first to admit, I struggled with anatomy in school; everyone does at first. But it is the absolute foundation of safe and effective manual and injection-based medicine. You have to be able to visualize the layers of tissue your needle is passing through.
Dosage Considerations
The volume of fluid injected also varies by region. The smaller muscles of the cervical and upper thoracic spine can comfortably accommodate about 1 mL of the solution per injection site. Injecting more than this can cause excessive pressure and discomfort. The larger, bulkier muscles of the lumbar region can easily handle 2 mLs per site, allowing for more thorough infiltration of the therapeutic solution throughout the trigger point complex.
It’s also important to note that you don’t always have to use the entire amount drawn into the syringe for a single patient. If there is any leftover Sarapin solution, for example, it cannot be saved for another patient or another procedure. However, some products, depending on their formulation and packaging, might be sent home with the patient for topical application if appropriate, but this is highly specific to the product and is not standard practice for injectable solutions.

The goal is to intentionally create a mild, localized inflammatory response to stimulate the body’s natural healing cascade, encouraging the growth of new, stronger connective tissue. It is primarily used to treat joint instability and ligament laxity. Sometimes, ozone is used in conjunction with this, a practice known as Prolozone.
- Platelet-Rich Plasma (PRP): This is a more advanced regenerative therapy. A sample of the patient’s own blood is drawn and spun in a centrifuge to concentrate the platelets.
