Ultrasound-Guided Barbotage for Calcific Tendinopathy Overview
Table of Contents
Welcome to our exploration of advanced treatments for chronic joint pain. In this post, I will guide you through the intricacies of calcific tendinopathy, a painful condition where calcium deposits form within tendons, most commonly in the shoulder’s rotator cuff. We will explore an innovative, minimally invasive procedure known as ultrasound-guided barbotage, a technique that breaks down and aspirates these calcium deposits to provide significant pain relief.
This discussion is based on the pioneering work of leading researchers in the field, whose findings I am privileged to share with you. I will explain the two primary methods—the single-needle and double-needle techniques—and provide a detailed, step-by-step walkthrough of the procedure, enhanced by real-time procedural video. Furthermore, I will explain how we integrate this advanced intervention with our comprehensive, multidisciplinary approach at Injury Medical Clinic, which combines chiropractic care, functional medicine, and medical oversight to promote optimal healing and long-term wellness.
At Injury Medical Clinic, PA, we have built a practice founded on the principle of integrative care. My journey in healthcare, reflected in my credentials (DC, APRN, FNP-BC, CFMP, IFMCP, ATN, CCST), has shown me that the most effective path to healing addresses the whole person, not just isolated symptoms. This philosophy is the cornerstone of our collaboration with Dr. Maria Guadalupe Cardenas, MD. Dr. Cardenas, a board-certified Internist with over four decades of clinical experience, serves as our esteemed Medical Director and Collaborative Physician. Her extensive medical knowledge provides the essential oversight that allows our clinic to offer a broad spectrum of care, from advanced medical procedures to holistic wellness strategies.
Our team works in a multidisciplinary model, where my expertise in chiropractic care and functional medicine, as a Family Nurse Practitioner, complements Dr. Cardenas’s deep understanding of internal medicine. This synergy enables us to manage a wide range of conditions, including complex personal injury cases, chronic pain syndromes, and metabolic disorders. By combining chiropractic adjustments to restore biomechanical function, functional medicine to identify root-cause imbalances, and targeted medical interventions under Dr. Cardenas’s direction, we create a personalized and robust treatment plan for every patient. This integrated framework ensures that when we consider procedures like barbotage, we do so within a comprehensive context aimed not just at symptom relief but at true, sustainable healing.
Before we explore the barbotage procedure, it is crucial to understand the condition it treats: calcific tendinopathy. This disorder involves the formation of calcium hydroxyapatite deposits within a tendon. While it can occur in various locations, such as the patellar tendon in the knee or the gluteus medius tendon in the hip, it is most commonly diagnosed in the supraspinatus tendon of the shoulder’s rotator cuff.
These calcium deposits can be in different phases:
Patients in the resorptive phase often present with acute, excruciating shoulder pain that can disrupt sleep and daily activities. The goal of treatment is to alleviate this pain and facilitate the removal of the calcium deposit. Ultrasound-guided barbotage is a highly effective, evidence-based intervention designed to achieve precisely that. The image below shows a classic example: you can see the skin, the deltoid muscle, and the supraspinatus tendon. Within the tendon is a bright, white calcium deposit. Notice the dark acoustic shadowing beneath it; this is a key diagnostic marker on ultrasound, as the dense calcium blocks the ultrasound waves from passing through.
Barbotage, derived from the French word for “to splash” or “to bubble,” is a procedure of repeated injection and aspiration used to break down and remove these painful calcium deposits. The entire procedure is performed under real-time ultrasound guidance, allowing precise needle placement and ensuring both safety and effectiveness. By visualizing the needle, the calcium deposit, and surrounding anatomical structures, we can target the problem area while avoiding injury to healthy tendon tissue and nearby neurovascular structures.
The primary objective is to decompress the tendon by removing the calcific material, which immediately reduces the intense pressure and inflammation responsible for the patient’s pain. This technique is a powerful alternative to more invasive surgical options.
Two primary methods exist for performing barbotage, and the choice often depends on the practitioner’s preference and the characteristics of the calcium deposit.
This method creates a “washing circuit” to flush out the calcium.
The single-needle technique, which I will detail in the procedural walkthrough below, is also highly effective. It involves using a single, larger-gauge needle to both inject saline and aspirate the dissolved calcium. This method requires repeatedly repositioning the needle within the deposit to target different areas, injecting fluid to create a slurry, and then aspirating it back into the same syringe.
To help you visualize this advanced technique, let’s walk through the single-needle barbotage procedure, as expertly performed by my colleague, Dr. Nate Nye, whose work provides an excellent demonstration of the clinical application.
The first and most critical step is ensuring the patient is comfortable and the procedure is performed under sterile conditions.
Once the area is numb, the main part of the procedure begins.
After the barbotage is complete (a process that can take 5-10 minutes of active work), it is common to perform a final injection.
At our clinic, treatment does not end when the needle is removed. The barbotage procedure is a powerful tool for removing the primary pain generator, but comprehensive recovery requires addressing the underlying biomechanical and physiological factors that may have contributed to the tendinopathy in the first place. This is where our integrative model shines.
By combining the precise, targeted intervention of ultrasound-guided barbotage with the holistic, foundational support of chiropractic, functional medicine, and rehabilitation, we offer a truly comprehensive solution for calcific tendinopathy. This approach, overseen by Dr. Cardenas’s medical expertise, ensures our patients receive the highest standard of care on their journey back to an active, pain-free life.
