Nerve Hydrodissection for Chronic Pain Solutions

In this discussion, we will explore the advanced technique of ultrasound-guided hydrodissection. This technique treats chronic elbow and forearm pain caused by radial nerve entrapment. Importantly, this advanced procedure represents a significant leap forward in treating nerve compression syndromes.
It often helps patients who fail to respond to conventional therapies. We will explore the underlying anatomy, procedural mechanics, and physiological rationale for using fluid to decompress nerves. A patient often comes to me with pain on the outside of their elbow, having been told they have “tennis elbow,” or lateral epicondylitis. While this is a common diagnosis, it’s not always the correct one. True lateral epicondylitis involves inflammation or degeneration of the tendons that attach to the lateral epicondyle, the bony bump on the outside of the elbow. However, when the pain persists despite standard treatments, or if it presents with unique characteristics, we must look deeper.
One such condition is Radial Tunnel Syndrome, a nerve entrapment neuropathy involving the radial nerve. This is precisely the scenario we will explore. We will do so based on the work of leading researchers in the field.
In a case, a patient experienced persistent elbow pain for over six months. The key diagnostic clue was the location and nature of the pain. It wasn’t at the lateral epicondyle itself but was located more distally on the back (dorsal aspect) of the forearm. The patient described it as a dull, burning pain. It worsened with specific movements like pronation and supination (twisting the forearm, like turning a doorknob). This clinical picture strongly suggests nerve involvement rather than simple tendonitis.
A diagnostic nerve block—an injection of a small amount of lidocaine near the suspected site of nerve compression—provided the patient with temporary relief. This positive response confirmed our suspicion. The radial nerve was the source of the pain. With this information, we could confidently proceed with a more definitive therapeutic intervention: hydrodissection.
To understand hydrodissection, we first need to appreciate the intricate anatomy of the elbow and forearm. The radial nerve travels down the arm. Near the elbow, it splits into two main branches: a superficial sensory branch and a deep motor branch, also known as the posterior interosseous nerve (PIN).
The PIN travels through a narrow passageway called the radial tunnel. A key structure within this tunnel is the Arcade of Frohse, a fibrous arch at the leading edge of the supinator muscle. This is a common site of nerve compression. As the nerve passes through this tight space, it can become entrapped by surrounding muscles, fascia, or fibrous bands. This can cause inflammation, restricted blood flow, and pain.

Hydrodissection is an advanced, minimally invasive procedure designed to treat nerve entrapment syndromes. The core principle is to use the gentle hydraulic pressure of a fluid to separate a compressed nerve from the surrounding tissues (fascia, muscle, or scar tissue) that are causing the entrapment.
The term “hydrodissection” literally means “dissection with water” (or, in this case, a saline or anesthetic solution). The goal is not to cut tissue but to carefully and precisely create space around the nerve.
Why is this technique so effective?
This procedure requires a profound understanding of anatomy as seen on ultrasound and exceptional needle-handling skills. It is not a technique for beginners. Therefore, clinicians must have specific training and experience, and it should be performed under expert guidance.
Let’s walk through the procedure as demonstrated by researchers, which mirrors the technique I use in my practice. The patient is positioned comfortably, and the skin over the target area is sterilized.
The entire process is performed with precise, controlled movements. Ultrasound feedback lets me see the nerve separating from the constricting tissue in real time. This confirms a successful dissection.
Hydrodissection is a powerful tool for relieving nerve compression, but it is one part of a larger recovery puzzle. True, long-lasting healing requires addressing the underlying biomechanical and functional issues that contributed to the problem in the first place. This is where integrative chiropractic care becomes essential.
