Abstract

A concussion or whiplash injury can affect much more than the painful area. The brain, digestive system, spinal nervous system, immune system, muscles, metabolism, and hormones constantly communicate. This relationship is often called the gut-brain axis. In integrative care, it can also help to consider a broader gut-brain-spine connection.

After a head or neck injury, inflammation, pain, poor sleep, stress, reduced activity, and changes in appetite can affect the body’s recovery environment. A functional medicine approach looks at these connected problems rather than focusing only on symptoms.

Concussion

A concussion is a type of mild traumatic brain injury. It can happen when the brain moves rapidly inside the skull because of a blow, jolt, or sudden acceleration and deceleration.

Research suggests that traumatic brain injury may disrupt normal communication along the gut-brain axis and cause changes in the intestinal microbiome and intestinal barrier (Iftikhar et al., 2020).

A major review of the microbiome, intestinal barrier, and brain also explains how gut microorganisms can create substances that interact with immune, metabolic, and neurological pathways (Gwak & Chang, 2021).

This is important because recovery after an injury requires more than repairing a painful muscle.

The body also needs to regulate inflammation, produce energy, repair tissues, maintain neurological function, and support the immune system.

Why Nutrition Matters During Concussion Recovery

The brain needs a large and steady supply of energy.

After a concussion, normal brain energy metabolism may temporarily change. At the same time, patients may eat less because they are experiencing nausea, fatigue, headaches, medication effects, or poor appetite.

A systematic review by Finnegan et al. (2022) examined nutritional interventions for mild traumatic brain injury.

Platelet-rich plasma uses a concentrated preparation produced from a patient’s own blood.

Researchers have studied PRP for several musculoskeletal problems.

There is also early research involving persistent cervical facet pain after whiplash.

Smith et al. (2022) reported improvements in pain and disability following cervical facet PRP injections in a prospective case series of patients with chronic whiplash-associated disorders.

Follow-up research also reported encouraging longer-term outcomes (Smith et al., 2023).

However, these were not large randomized clinical trials.

PRP should therefore be described as a developing treatment option for carefully selected structural problems, rather than a guaranteed treatment.

It should also not be described as regenerating the brain after concussion.


Laser and Shockwave Therapy

Photobiomodulation, sometimes called therapeutic laser treatment, uses light energy to influence cellular processes.

There is active research into photobiomodulation and traumatic brain injury. However, much of the evidence for direct brain treatment remains experimental, and strong human clinical data are still limited (Stevens et al., 2023).

Laser therapy has also been studied in musculoskeletal care.

Shockwave therapy uses acoustic energy and has been investigated for several tendon and soft-tissue conditions. Studies have also examined its use for neck and shoulder myofascial pain.

These treatments may have a role in selected cases, but the specific injured tissue and diagnosis should determine whether they are appropriate.
