Frozen Shoulder Relief: The Role of Hydroplasty in Treatment
Table of Contents
As a clinician and researcher dedicated to bringing you the latest in evidence-based musculoskeletal care, I, Dr. Alex Jimenez, am excited to share insights into a powerful technique for managing one of the most stubborn and painful conditions we see: adhesive capsulitis, more commonly known as frozen shoulder. This condition can severely limit a person’s quality of life, making even simple tasks feel monumental. Today, we will explore an advanced, minimally invasive procedure called hydroplasty, also known as hydrodistension, which offers real hope for those living with this debilitating condition.
We will explore the physiological basis of frozen shoulder and why hydroplasty is such an effective intervention. I will present the work of leading experts, like my colleague Dr. Paul Taylor, a primary care sports medicine physician, who has refined this technique. We will break down the procedure step by step, from the initial nerve block to the final distension of the joint capsule, and examine the science behind each component.
Furthermore, I will explain how this advanced procedure integrates seamlessly into our multidisciplinary care model here at the Injury Medical Clinic PA. Our unique approach combines my expertise in chiropractic, functional medicine, and advanced practice nursing with the invaluable medical oversight of Dr. Maria Guadalupe Cardenas, MD. Dr. Cardenas, a board-certified internist with over 40 years of experience, serves as our Medical Director and Collaborative Physician. Together, our team provides a comprehensive treatment spectrum that includes functional medicine, rehabilitation, and personal injury care, ensuring each patient receives a holistic, personalized recovery plan. This post shows how we use hydroplasty as a cornerstone of treatment, followed by targeted chiropractic care and rehabilitation to restore mobility, reduce pain, and help you get back to living your life to the fullest.
Adhesive capsulitis is an insidious condition characterized by severe pain and a progressive loss of shoulder motion. It occurs when the glenohumeral joint capsule—the soft tissue envelope surrounding the ball-and-socket joint of the shoulder—becomes inflamed, thickened, and fibrotic. This process essentially “shrinks” the capsule, restricting movement and causing significant pain.
The condition typically progresses through three stages:
Traditional treatments, such as physical therapy and anti-inflammatory medications, can be helpful but are often slow and frustrating for patients. This is where more advanced interventions like hydroplasty come into play, offering a way to accelerate the “thawing” process and provide more immediate relief.
At Injury Medical Clinic PA, we believe that complex conditions like frozen shoulder require a multifaceted approach. As a Doctor of Chiropractic (DC) and Advanced Practice Registered Nurse (APRN), I can view the patient’s condition through both a biomechanical and systemic lens. I focus on restoring proper joint mechanics, addressing nerve function, and guiding rehabilitation.
This is complemented by the essential role of our Medical Director, Dr. Maria Guadalupe Cardenas, MD. As a seasoned internist, Dr. Cardenas provides crucial medical oversight, manages pharmacological interventions, and ensures all procedures are performed safely and effectively. Her NPI number is 1164426749, and she is licensed in Texas under #J2933. This collaborative model, where a chiropractor and a medical doctor work side by side, is the bedrock of modern integrative care. It allows us to offer a comprehensive suite of services under one roof, including:
This integrated team approach ensures we are not just treating the shoulder but addressing the whole person, leading to more robust, lasting outcomes.
Hydroplasty, or hydrodistension, is a procedure designed to physically stretch and break up the adhesions within the shrunken joint capsule. The goal is to mechanically expand the capsule from the inside out using a large volume of sterile fluid. This directly counteracts the pathological process of adhesive capsulitis, creating space within the joint and paving the way for improved mobility.
To illustrate this technique, I want to showcase the work of Dr. Paul Taylor, a primary care sports medicine physician with OrthoVirginia. His meticulous, ultrasound-guided approach represents the gold standard for this procedure. Let’s look at the anatomical target using an ultrasound image.
Dr. Taylor’s method is a carefully orchestrated, three-step process designed for maximum efficacy and patient comfort. The entire procedure is performed under ultrasound guidance, allowing precise needle placement and real-time visualization of the joint, which dramatically increases safety and success rates.
The first and arguably one of the most crucial steps is to provide effective pain control. Frozen shoulder is intensely painful, and the procedure itself involves distending an already sensitive structure. To manage this pain, the surgeon performs a suprascapular nerve block.
From my clinical observation, this initial nerve block is a game-changer. It turns a potentially painful procedure into a tolerable one and lets the patient do post-procedure mobility exercises with much less discomfort.
Next, before introducing the large-volume needle, the pathway and the joint capsule itself are anesthetized.
This is the core of the procedure. Here, the goal is to inject a large volume of fluid to stretch and ultimately rupture the fibrotic adhesions.
The physician and assistant can feel the resistance build as the capsule stretches. Then, suddenly, there is a “give” or a release of pressure as the adhesions tear. On the ultrasound, we can often see the capsule expand and then partially deflate as the fluid finds new space or extravasates through the micro-tears. This moment signifies a successful procedure—the mechanical restriction has been broken.
The hydroplasty procedure is a powerful catalyst for recovery, but it is not the end of the story. The “thaw” has been initiated, but now the patient must reclaim their range of motion. This is where integrative chiropractic care becomes essential.
Immediately following the procedure, while the shoulder is still numb from the nerve block, we begin gentle mobilization. My role is to guide the patient through a specific protocol to capitalize on the joint’s new freedom.
This combination of an advanced medical procedure followed by targeted chiropractic and rehabilitative care creates a powerful synergy. Hydroplasty breaks down the primary physical barrier, and the subsequent therapy retrains the body to use its newfound mobility correctly, supporting a complete and lasting recovery. By working collaboratively under one roof, Dr. Cardenas and I can coordinate this care seamlessly, providing our patients in El Paso with a truly comprehensive solution for frozen shoulder.
