BHRT for Joint Stiffness, Mobility, and Flexibility

Abstract

Bioidentical hormone replacement therapy (BHRT) may indirectly help mobility and flexibility when declining hormone levels contribute to joint stiffness, reduced bone density, muscle weakness, fatigue, or discomfort. Estrogen, progesterone, and testosterone affect many tissues involved in movement, but BHRT should not be viewed as a direct flexibility treatment. A complete approach may combine medically supervised hormone care with exercise, nutrition, rehabilitation, and integrative chiropractic care. Can BHRT Help You Move and Feel More Flexible?

As people age, they may notice that their bodies do not move as easily as they once did. Getting out of bed may take longer. The hips may feel tight. The knees may ache after sitting. The shoulders may not move through the same range of motion.

These changes do not always come from a single problem.

Movement can be affected by:

  • Hormone changes
  • Joint stiffness
  • Muscle weakness
  • Reduced physical activity
  • Bone loss
  • Previous injuries
  • Poor posture
  • Inflammation
  • Sleep problems
  • Changes in body composition

Bioidentical hormone replacement therapy may help some of these factors when a person has a medically identified hormone imbalance or deficiency.

However, BHRT is not a direct treatment for flexibility. Stretching, exercise, strengthening, rehabilitation, and appropriate musculoskeletal care are still needed to improve movement.

The best way to think about BHRT is that it may help create a healthier internal environment that makes regular movement and rehabilitation easier.

What Is Bioidentical Hormone Replacement Therapy?

Bioidentical hormones are hormones that have the same basic chemical structure as those naturally produced by the human body.

Examples may include:

  • Estradiol
  • Progesterone
  • Testosterone

Hormone therapy may be considered when natural hormone levels fall, and a person develops symptoms that affect health and quality of life.

During menopause, for example, estrogen levels decline. Men can also experience lower testosterone levels as they age or because of medical conditions.

These hormone changes may affect much more than reproductive health. They can also influence bones, muscles, connective tissues, sleep, energy, and physical activity.

Mayo Clinic explains that the word bioidentical does not automatically mean a hormone product is safer than other forms of hormone therapy. FDA-approved bioidentical hormone medications are available, while compounded products may have different levels of testing and regulation (Mayo Clinic, 2024).

How Hormones Affect Joints and Movement

Hormones interact with many tissues throughout the body.

Estrogen, for example, is involved in:

  • Bone maintenance
  • Connective tissue function
  • Muscle health
  • Inflammatory signaling
  • Joint-related biological processes

When estrogen falls during menopause, some women notice more joint aching or stiffness.

Research has shown an association between menopause and a group of musculoskeletal changes involving bones, joints, muscles, and connective tissues. These changes have sometimes been described as the musculoskeletal syndrome of menopause (Wright et al., 2024).

This does not mean that hormones cause every joint problem during menopause.

Arthritis, old injuries, poor movement patterns, obesity, spinal problems, and other conditions may also contribute.

Still, hormone changes may be one factor.

BHRT May Help Reduce Joint Discomfort

One possible way hormone therapy can support mobility is by reducing joint discomfort in certain patients.

A large Women’s Health Initiative study found that women taking estrogen alone experienced a modest reduction in joint pain compared with women taking a placebo (Chlebowski et al., 2013).

The benefit was not dramatic, and estrogen should not be considered a standard pain medicine.

However, even a small reduction in discomfort can sometimes make it easier for a person to:

  • Walk
  • Exercise
  • Stretch
  • Perform strengthening exercises
  • Participate in rehabilitation
  • Stay active throughout the day

This creates an important connection.

If hormone management helps someone feel comfortable enough to become more active, that increased activity may then improve mobility.

Patient education from BodyLogicMD and Maven Clinic also discusses the possible relationship between hormone replacement therapy, menopause, and joint discomfort (BodyLogicMD, 2025; Maven Clinic, 2026).

Does BHRT Improve Flexibility?

BHRT does not directly stretch muscles or increase joint range of motion.

Flexibility normally improves through repeated physical movement.

This can include:

  • Gentle stretching
  • Mobility exercises
  • Strength training
  • Yoga or controlled movement
  • Rehabilitation exercises
  • Walking
  • Proper warm-ups
  • Consistent physical activity

Hormone therapy may make these activities easier if it improves fatigue, weakness, stiffness, poor sleep, or joint discomfort.

That is why the relationship between BHRT and flexibility is mostly indirect.

For example, someone with less joint discomfort may feel more willing to walk and stretch. Over time, regular movement can help improve joint motion and muscle flexibility.

Estrogen and Bone Health

One of the clearest benefits of menopausal hormone therapy involves bone health.

Bone density naturally becomes a greater concern after menopause because estrogen plays an important role in maintaining bone.

The North American Menopause Society, now known as The Menopause Society, states that hormone therapy can help prevent bone loss and fractures in appropriately selected women (The North American Menopause Society, 2022).

Research from the Women’s Health Initiative also found that estrogen combined with progestin increased bone mineral density and reduced fracture risk in postmenopausal women (Cauley et al., 2003).

Healthy bones are important for long-term mobility.

Stronger bones may help people feel more confident when

  • Walking
  • Resistance training
  • Recreational activities
  • Balance exercises
  • Daily household tasks
  • Physical rehabilitation

BHRT does not make a person flexible simply by improving bone health. But protecting the skeleton can support a person’s ability to remain active for many years.

Hormones and Muscle Strength

Muscle strength is another important part of mobility.

Strong muscles support the joints, stabilize the spine, improve balance, and make daily activities easier.

For postmenopausal women, hormone therapy has not consistently been shown to increase muscle mass significantly.

A systematic review by Javed et al. (2019) found that menopausal hormone therapy did not produce a significant overall improvement in lean body mass.

This means exercise remains essential.

For men with medically diagnosed testosterone deficiency, testosterone replacement may have a clearer effect on muscle strength.

A systematic review found that testosterone replacement therapy may improve muscle strength in older men who have relatively low testosterone levels (Lee et al., 2023).

Still, hormone treatment should not replace:

  • Resistance exercise
  • Protein intake
  • Proper nutrition
  • Sleep
  • Rehabilitation

Hormones and exercise work through different pathways.

What About Cartilage and Inflammation?

Some online discussions about BHRT claim that hormone therapy directly rebuilds cartilage or repairs damaged joints.

That claim goes too far.

Hormones may influence inflammation and connective tissue biology, but BHRT has not been proven to regenerate worn or damaged cartilage.

People with:

  • Osteoarthritis
  • Torn ligaments
  • Tendon injuries
  • Cartilage injuries
  • Disc problems
  • Significant joint degeneration

may require additional evaluation and treatment.

Reducing discomfort or improving general wellness may help the person become more active, but structural injuries still need proper musculoskeletal care.

Why Integrative Chiropractic Care Can Complement Hormone Treatment

Hormone therapy focuses mainly on biological and metabolic factors.

Integrative chiropractic care addresses another important part of the problem: how the body moves mechanically.

A person may have balanced hormone levels but still struggle with:

  • Restricted spinal motion
  • Poor posture
  • Weak stabilizing muscles
  • Hip stiffness
  • Shoulder restrictions
  • Muscle imbalances
  • Previous injuries
  • Poor movement patterns

This is where chiropractic care and rehabilitation can become important.

Research suggests that manipulation and mobilization may help reduce pain and improve function in some patients with chronic low-back pain (Coulter et al., 2018).

The goal is not to claim that chiropractic care changes hormone levels. Instead, chiropractic care can address movement restrictions while medical providers address hormone-related concerns.

An integrative approach asks two different questions.

First: What is happening inside the body?

This may involve looking at:

Is BHRT Safe for Everyone?

No.

Hormone therapy must be individualized.

A person’s medical provider may consider factors such as:

  • Age
  • Symptoms
  • Medical history
  • Family history
  • Time since menopause
  • Cardiovascular risk
  • Blood clot risk
  • Cancer history
  • Type of hormone
  • Dose
  • Route of administration

Mayo Clinic explains that bioidentical hormones are not automatically safer simply because they are described as natural or identical to human hormones (Mayo Clinic, 2024).

The Menopause Society also recommends personalized medical decision-making because the benefits and risks of hormone therapy differ from one person to another.

BHRT should therefore be prescribed and monitored by a qualified healthcare professional.

The Bottom Line

So, can bioidentical hormone therapy help mobility and flexibility?

It may help indirectly.

For appropriately selected patients, hormone therapy may improve factors that make movement difficult, such as joint discomfort, bone loss, fatigue, or certain hormone-related changes in muscle function.

But BHRT does not directly stretch tight muscles, correct poor posture, restore restricted joints, or replace exercise.

That is why an integrative plan can make sense.

Hormone management may address the biological side of movement, while chiropractic care, exercise, and rehabilitation address the mechanical side.
